Our Vision & Mission

Vision

Improving Access to Healthcare, Neighbourhood by Neighbourhood

Mission

NHS aims to act as the bridge to reconnect residents-in-need with the healthcare system by helping them overcome barriers to healthcare access, through a comprehensive health screening as well as targeted follow-up and education programmes.

Our Approach

Neighbourhood Health Service (NHS) aims to identify and engage residents who are disconnected from the healthcare system, particularly those who have fallen through care gaps and remain underserved. By serving as a bridge to healthcare services, NHS helps reconnect these residents to appropriate care.

Phase 1 — Pre-Screening: Community Engagement and Outreach
Phase 1

Door-to-door outreach to identify and invite residents who have fallen through care gaps.

Phase 2 — Screening: Centralised, Multi-Modality Access and Risk Identification
Phase 2

Centralised multi-modality health screening to assess risk and identify undiagnosed conditions.

Phase 3 — Post-Screening: Tiered Follow-Up and Longitudinal Support
Phase 3

Tiered follow-up and longitudinal support to reconnect flagged residents with appropriate care.
